WTOHT Approved as Ontario Health Team

In September, the West Toronto Ontario Health Team (WTOHT) submitted our full application to become an Ontario Health Team. We are pleased to announce that our application has been reviewed and approved. We look forward to moving ahead to serve the residents of West Toronto by creating a seamless integration of health services that meets the needs of patients, clients, families and caregivers to receive coordinated care and strengthens the response of providers.
