Healthy Neighbourhoods 

  • Anti-Black Racism 

A collaborative team formed with Mississauga Ontario Health Team to address Anti-Black Racism in health and social services.

  • Cancer Screening 

West Toronto Ontario Health Team Task Force aiming to increase cancer screening rates in the community. Our team aims to address the factors creating barriers to cancer screening.  

Primary Care Access 

Primary care access and even the model of primary care clinic accessed by patients significantly influences cancer screening rates. Our team is working to increase cancer screening rates for those who are unattached or uncertainly attached to primary care. 

Social Determinants of Health 

The social determinants of health, including factors like income and race, are predictive of health outcomes and health service access. Our team is actively engaged in planning and running community outreach campaigns to reach high priority neighbourhoods that see disproportionately lower cancer screening rates. 

  • Climate & Health 

As a result of the climate crisis, West Toronto has seen extreme heat and worsening air quality each summer. These environmental factors have outsized impacts on older adults, individuals managing chronic illnesses, individuals living without access to air conditioning, housing insecure or unhoused individuals, and more. The long-term impacts of the climate crisis are also the source of significant mental health concerns, especially for youth and young adults. 

Our team has implemented community-based interventions like climate resilience kits, enhanced cooling centres, and mental health programming to help build climate resilience for better health in West Toronto. 

  • Integrated Primary Care 

Access and attachment to primary care is one of the most important challenges that we must address in West Toronto. Ensuring every resident in West Toronto has access to an interprofessional primary care team means working with primary care providers, allied health professionals, and patients to transform how primary care works in our community. 

Our team’s first initiative in this space is the creation of an integrated health access model embedded in Toronto Community Housing locations (iHelp Centres) to facilitate access and attachment in high-priority neighbourhoods.
