Our vision:
The Mental Health and Addictions (MHA) Working Group seeks to ensure mental health and addiction services are readily available and equitably meet the needs of residents within West Toronto.
What we’re doing:
Based on the data our team has collected, we have concluded that many individuals requiring mental health and addictions services are going to the emergency department for services and are unsure of how and where to access these services in the community.
We have developed a plan to streamline access for patients/clients with mild to moderate mental health and addictions needs ensuring individuals know where to receive services in the community across multiple providers in primary care and other community settings. We are piloting how we can effectively enable this coordination of services digitally by providing information on how to access services and where the services are located.
We are testing this project in the neighbourhood of South Parkdale and developing a basket of services that would be available to this priority population. Following this test of change, we will be spreading our learnings to other West Toronto neighbourhoods.
