Ontario Health Teams are part of a provincial government initiative dedicated to building a connected health care system. The West Toronto Ontario Health Team is a consortium of 35 organizations in the West Toronto area which helps coordinate care between hospitals, primary care, home and community care services, long-term care homes, congregate settings, and other services to deliver an improved and seamless healthcare experience for those receiving care in West Toronto. The West Toronto Ontario Health Team seeks to make health and social care delivery equitable through their vision of working collaboratively with patients, caregivers and the community.

Why Join the WTOHT?
As a member of the WTOHT, your organization will become part of a collaborative group of organizations, working together with patients and caregivers to equitably improve the experiences and outcomes of people living and seeking care in West Toronto. Led by our guiding principles, we aim to work together to become one connected system for health and social care.
Here are some key benefits of joining the WTOHT:
participate in WTOHT annual planning to co‐design and deliver on the vision of the WTOHT;
shape the WTOHT approach to care;
participate in and commit to implementing OHT service delivery initiatives including process changes from collaborative Quality Improvement Plan (cQIP) initiatives;
participate in the selection of the Steering Committee members;
contribute to working groups;
partake in a wide range of engagement and co‐design activities so their perspectives and experiences are represented in the work of the WTOHT; and
participate in regular meetings and updates
