West Toronto OHT – Digital Framework

Vision: To improve the experiences and outcomes of patients/clients/residents, caregivers and healthcare providers in the West Toronto OHT through advancing connected care and enabling equitable access to digital solutions and/or alternatives.

There are two components to our Digitally Integrated Health System

Patients/Clients/Residents & Caregivers Support the digital enablement of patients/clients/residents and their caregivers to make the healthcare choices that are right for them.

Healthcare Providers
Support and connect providers to leverage digital tools and the resources they need to provide care for West Toronto OHT’s population.
