Digital Planning

Understand use of provincial and regional digital assets by OHT partners, to inform digital planning.

Support digitalization of primary care.

Advocate for WTOHT’s digital needs at provincial and regional tables.

Collaborate with other OHTs to identify opportunities to evolve WTOHT’s digital maturity.

Digital Funding Opportunities

Support WTOHT’s applications for digital funding opportunities released by Ontario Health (OH). If successful, support planning and implementation of the initiatives. Current successful applications being supported are:

• Primary Care Online Appointment Booking for Patients

• Test of Change for a Digitally Enabled Shared Integrated Care Pathway

Support Population Health Management and Patient Navigation

Support WTOTH’s work in identifying and tracking its priority patient populations and 24/7 patient navigation, through leveraging digital assets.

Patient Access to Digital Health Information

Collaborate with other Ontario Health Teams to identify requirements for a regional solution that enables patients to access their own health information digitally.

Test of Change: Digital Pilot

Collaborate with the COPD Working Group to develop a digitally enabled integrated shared care pathway for care teams in West Toronto to support its COPD patient population.
