Our vision
The Frail Seniors’ Working Group seeks to ensure senior related services are readily available and equitably meet the needs of seniors within West Toronto.
What we’re doing
The team has developed a plan to ensure a common language and definition is used among health care providers to identify seniors, who need services, and to ensure that these seniors receive the care they need based on their needs.
Currently, this plan is being tested in the Mimico neighbourhood helping us identify improvements to be made, as well as bring together services across providers to meet the needs of the broader population of seniors in Toronto’s West end. Following this test of change, we will be spreading our learnings to other West Toronto neighbourhoods.
The WTOHT is also collaborating with seven other OHTs in Toronto to develop a centralized navigation phone and online service directory to streamline navigation options for seniors across Toronto. The goal is to improve patient access to care through better navigation and stronger service connections. A Community Advisory team will be supporting this important work, with representation from the patient population in co-designing this system.
